Consent to personal data processing

By submitting any form on this website, you confirm that you consent to the processing of your personal data under the terms set out below.

Bloom Reproductive Agency LLC (Georgian: ბლუმ რეპროდუქციული საგენტო), identification code 405864042, address: Georgia, Tbilisi, Shartava St. N3, apt. 141. “Bloom Fertility Agency” is a trade name. Data-related contact: Hello@bloomfertilityagency.com.

2. Categories of data

  • Identification data (name, date of birth)
  • Contact data (phone, email, country of residence)
  • Application data — for donor applications: appearance details and a self-report on health; at later stages, extended medical and biographical data, photos and video
  • The text of your enquiry and correspondence with us

3. Purposes of processing

  • Reviewing your application or enquiry and contacting you
  • Assessing eligibility for the programme (for donors and surrogates)
  • Coordinating medical screening and the programme
  • Legal and accounting support

4. Recipients of data

  • Agency staff with access on a need-to-know basis
  • Partner clinics and intended parents — only within the public profile, without direct contact details
  • Lawyers — to the extent needed to formalise a specific programme
  • Data is not shared with third parties for marketing purposes

5. Storage and protection

Data is kept in a secure system with restricted access for the duration of the application review and programme participation, and for 3 (three) years after the last cycle or end of participation — for medical and legal record-keeping, or longer where required by law.

6. Your rights

  • Request access to your data and its correction
  • Request deletion of your data (except where storage is required by law or for medical/legal record-keeping)
  • Withdraw consent at any time by writing to Hello@bloomfertilityagency.com — withdrawal does not affect the lawfulness of processing before withdrawal, but may mean you cannot continue in the programme

7. Voluntary nature

Providing data is voluntary but necessary to review your application or enquiry. Without it, the agency cannot process your request.
